Need a doctor?

Do you need a doctor or dentist for your child’s back-to-school check-up? What does your child need to be ready for school? If you don’t have a family doctor or dentist, if you don’t have insurance, or if you can’t get an appointment that works–the Polk County Health Department has a new program to help connect families with medical providers.

Families of students in Polk County entering preschool through 12th grade, can call (515)323-5234 and a staff member will assist them in getting an appointment with a doctor or dentist, and with finding health insurance.

Calls will be answered Monday-Friday 8:00 am- 5:00 pm and extended hours on Tuesdays until 7:00 pm. Appointments start on July 27, 2015 and run through October 2, 2015.

Appointments and services are provided by the following partners: Polk County Health Department, Broadlawns, Des Moines University Clinic, Mercy Medical Center, Primary Health Care, Blank Children’s Hospital, and Visiting Nurse Services of Iowa.
