Preschool Registration
If you have any questions about the preschool registration process, and which forms are required, please call 515-242-7588.
Health Requirements
The school nurse partners with you to support your student’s health. Please contact the nurse if you need referrals for medical or dental services or to discuss your student’s health needs at school. All preschool students are required to turn in the following documents:

Signed Certificate of Immunization from the health care provider.

School Physical Exam within the past 12 months; including Hemoglobin and Lead blood test results from the health care provider.A current physical must be on file all the time. If the student visits their health care provider for a new physical during the school year, please provide a copy of the new physical to the school nurse.

Certificate of Dental Screening within the last 6 months from the dental care provider.
